About the Role

Responsible for coordinating and delivering physical therapy services, developing and implementing individualized treatment plans, and evaluating patient progress. Communicates effectively with patients, families, and interdisciplinary team members to ensure comprehensive care.

Responsibilities

• Evaluate patients through various data sources and develop personalized treatment programs.
• Utilize functional activities to promote health and achieve treatment goals, adjusting strategies as needed.
• Provide direct therapy services across different settings, considering patients' physical, emotional, and intellectual needs.
• Prepare documentation including evaluations, treatment plans, and progress notes.
• Maintain effective communication with patients, families, physicians, and team members.
• Collaborate with an interdisciplinary team and participate in ongoing professional development.

Requirements

• Active license as a Physical Therapist in the practice state.
• Preferred 1-3 years of licensed physical therapy experience.
• Strong communication and visual acuity.
• Ability to work effectively in a team environment.
• Commitment to professional growth and adherence to safety and confidentiality standards.
