About the Role

As a Physical Therapist (PT) at York Nursing and Rehab Center, you will have the opportunity to make a positive impact on the lives of our patients. As a key member of our rehabilitation team, you will be responsible for providing high-quality physical therapy services to patients with varying levels of physical disability. Your expertise will help patients regain strength, mobility, and functional ability, enabling them to return to their normal daily activities.

Responsibilities

• Design and implement individualized physical therapy treatment plans to meet the unique needs of each patient
• Conduct patient evaluations and assessments to identify areas of strength, weakness, and dysfunction
• Develop and implement therapeutic exercises, stretches, and activities to improve mobility, strength, and function
• Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal patient outcomes
• Document patient progress and treatment plans accurately and timely
• Participate in patient education and discharge planning to ensure smooth transitions to the next level of care

Requirements

• Bachelor's or Master's degree in Physical Therapy
• Current licensure as a Physical Therapist in the state of Pennsylvania
