Responsibilities
- Provide individual therapy services to residents according to care plans
- Assist in resident assessment activities as needed
- Develop therapy plans for individual residents in conjunction with MDS
- Keep the therapist informed of any need to adjust therapy plans to match or support resident care plan goals
- Brief residents on physical therapy procedures and prepare necessary equipment
- Participate in discharge planning, development and implementation of resident care plans, and resident assessments
- Interview residents and family members as necessary
- Coordinate therapy appointments with other departments
- Follow therapy department safety standards
Requirements
- Bachelor’s Degree in Physical Therapy from an accredited college or university (minimum requirement)
- Experience in a therapist capacity in a hospital, nursing care facility, or other related medical facility
- Training in rehabilitative and restorative therapy practices
- Current, unencumbered license to practice as a Physical Therapist in Pennsylvania
- Knowledge of physical therapy procedures
- Leadership and supervisory ability with willingness to work harmoniously with and supervise other personnel
- Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures of the physical therapy service
- Maintain care and use of supplies, equipment, and therapy areas
- Ability to relate information concerning a resident’s condition
- Successfully complete a post-offer, pre-employment criminal background check and physical/drug screen
