About the role

We are seeking a licensed Physical Therapist to provide individualized therapy services to patients, developing and implementing treatment plans based on thorough evaluations.

Responsibilities

• Evaluate patients through various data sources.
• Create and review personalized treatment programs and instruct patients and families.
• Utilize functional activities to promote health and adjust interventions as needed.
• Deliver direct therapy services across diverse settings and coordinate with healthcare teams.
• Maintain accurate documentation of evaluations, treatment plans, and progress notes.
• Communicate effectively with patients, families, and team members about patient status and needs.
• Supervise physical therapy assistants, technicians, and students to ensure safe, efficient care.
• Stay current with equipment use and professional development.

Qualifications

• Active license as a Physical Therapist in the practice state.
• Strong communication and interpersonal skills.
• Ability to perform essential functions, including visual acuity and hearing.

Preferred qualifications include:

• Licensed physical therapist with additional certifications or experience.
