Physical Therapist (PT), Home Health — Company Car + Gas Card | Edmond, OKC, Guthrie, Stillwater

About the role

Full-time home-health Physical Therapist position with a locally owned, Medicare-certified home health, hospice, and private-duty agency serving Guthrie, Edmond, and Stillwater. The role involves one‑on‑one patient visits in the field, using a company car and gas card, with pay per visit and expected full‑caseload earnings of $85,000‑$95,000 per year. You will set a field‑based weekday schedule, work independently in patients’ homes, and collaborate with nursing, therapy, and case‑management teams.

Responsibilities

• Evaluate and treat homebound patients across central Oklahoma (Oklahoma City, Edmond, Guthrie, Stillwater, Perry, and nearby communities).
• Build plans of care, teach patients and families home programs, and progress patients toward safe mobility at home.
• Coordinate with nursing, occupational therapy, speech therapy, and case management.
• Complete documentation within 24 hours of each visit using a provided tablet and Matrixcare software.

Requirements

• Licensed Oklahoma Physical Therapist, or eligible to obtain an Oklahoma license.
• Graduate of a CAPTE‑accredited PT program.
• Current CPR certification.
• Valid driver’s license.

Qualifications

• Home‑health experience is a plus but not required.
• Strong background in outpatient, hospital, or rehab PT settings is encouraged.
• New graduates with solid clinical internships will be considered; training on home‑health workflow will be provided.
