About the role
Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians that considers the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient.
Accomplishes reassessment of patients at appropriate intervals, documents progress, and determines the effectiveness of therapy.
Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician.
Instructs patients and caregivers on the use of assistive devices and establishes home programs.
Documents treatments accurately and timely in medical records and updates care plans as necessary.
Interacts and participates in the education, quality improvement activities, role development, and orientation of agency personnel, promoting and supporting growth of others through precepting and mentoring.
Collaborates with other healthcare professionals to coordinate and direct the care of home patients requiring physical therapy, ensuring comprehensive assessment, planning, implementation, and evaluation.
Prioritizes and adapts home care caseloads to meet the changing needs of patients and families.
Selects, applies, and modifies skilled interventions to achieve the best outcomes for patients, reassessing as needed.
Performs evaluations to assess patients' functional abilities, and documents results in clinical records.

Requirements
Must have a current license to practice as a Physical Therapist in the state of Colorado.
Must have a minimum of one year of experience in home health care setting.
Must have a strong understanding of home health regulations and third-party reimbursement processes.
Must possess excellent communication and interpersonal skills.
Must be able to work independently and manage multiple tasks simultaneously.
Must be able to travel to various locations within the service area.
Qualifications
Graduation from an accredited program in Physical Therapy.
Current licensure as a Physical Therapist in the state of Colorado.
At least one year of experience in home health care setting.
Skills

Pay
$40.00 - $45.00 per hour.
Schedule
Full time, Monday – Friday, 8am – 5pm, flexible, rotating weekends on-call.
