Schedule
Full-Time; M-F 40 hours with required weekend/holiday rotations as needed
What You'll Do
- Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
- Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
- Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
- Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.

What You'll Need
- Active Florida PT license
- Minimum of 1 year experience as a licensed Physical Therapist (PT).
- Home health care experience strongly preferred.
- Strong assessment, critical thinking, and patient education skills
