About the role
Joining AdventHealth is about extending the healing ministry of Christ. As a physical therapist, you'll be part of a community dedicated to the whole person, providing care that addresses physical, cultural, psychosocial, spiritual, age-specific, and educational needs.
Responsibilities
- Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians.
- Reassesses patients at appropriate intervals, documenting progress and determining the effectiveness of therapy.
- Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician.
- Instructs patients and caregivers on the use of assistive devices and establishes home programs.
- Documents treatments accurately and timely in medical records and updates care plans as necessary.
- Interacts and participates in the education, quality improvement activities, role development, and orientation of agency personnel.
- Coordinates and directs the care of home patients requiring physical therapy, ensuring comprehensive assessment, planning, implementation, and evaluation.
- Prioritizes and adapts home care caseloads to meet the changing needs of patients and families.
- Selects, applies, and modifies skilled interventions to achieve the best outcomes for patients, reassessing as needed.
Requirements
- Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing.
- Demonstrated knowledge of physical therapy theory, modalities, treatments, and skills.
- Basic computer skills and knowledge of Microsoft Office.
- Must be able to read, write, and speak conversational English.
Qualifications
- Doctorate in Physical Therapy (Field of study).
- 1+ year relevant clinical physical therapy experience.
- Recent, relevant experience in a Medicare-certified home health agency as a care-manager (Preferred).
Skills
- Knowledge of Home Care Regulations and Third-Party Reimbursement.
- Basic computer skills and knowledge of Microsoft Office.
- Ability to read, write, and speak conversational English.

Schedule
Full time: Monday – Friday, 8am – 5pm, flexible, rotating weekends on-call.
Pay
$34.71 - $64.55 per hour.
Additional Information
- Licenses and certifications required: Physical Therapist (PT), Driver's License (DL), Auto Insurance (AL), Basic Life Support - CPR Cert (BLS), Certified for Oasis Specialist-Clinical (COSC) (Preferred).
- Education requirements: Doctorate in Physical Therapy (Field of study).
- Background Screening Requirement: Certain positions are subject to Level 2 background screening, including fingerprinting, as required by state law.
