Physical Therapist (PT) - Home Health - Assisted Living Based

Responsibilities

• Supervise, evaluate, and provide therapy services to clients in various senior living communities.
• Provide therapy services to patients within the confines of assisted living facilities.
• Offer 1:1 patient care.
• Collaborate with assisted living facility's wellness team on a daily basis.
• Maintain strong lines of communication and overall continuity of care for residents and their loved ones.
• Examine and treat patients with physical impairments using physical modalities.
• Achieve patients' goals through programs like Balance and Fall Prevention, Spine Safety, Low Vision, and Diabetic Management.
• Administer skilled care to clients requiring intermittent professional services.
• Teach family and other members of the health care team about the provided care.

Qualifications

• Be currently licensed as a Physical Therapist (PT) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Show knowledge and skill in current therapy practice.
• Possess a valid state driver’s license and automobile liability insurance.
