About the Role

Deliver high-quality, patient-centered physical therapy services in accordance with physician orders, collaborating with healthcare team members and educating patients and caregivers on home exercise plans.

Responsibilities

• Conduct initial and ongoing assessments to evaluate mobility, strength, and function
• Administer, evaluate, and interpret tests to set functional goals and outcomes
• Provide written evaluations and plans of care outlining therapy needs, goals, and progress
• Deliver physical therapy services in accordance with physician orders
• Maintain accurate documentation, modifying treatment plans and planning discharges
• Collaborate with healthcare team members, educating patients and caregivers on home exercise plans
• Supervise personnel such as PTA, OTA, and HHA as applicable
• Complete assessments like OASIS where permitted by regulation

Qualifications & Skills

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license as a Physical Therapist
• Valid CPR certification
• Valid driver’s license and automobile insurance
• Effective documentation skills to ensure compliance and minimize denials
