Physical Therapist PT for Home Health

About the role

FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages.

Responsibilities

• Control your own schedule and work in your preferred coverage areas.
• Treat patients when and where you want, with no minimum requirement.
• Determine your own compensation based on the number of patients you treat.
• Receive administrative support from FeldCare Connects for assigning, communication, scheduling, care coordination, and quality assurance.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Use our innovative app Cliniconnects to accept or decline patients in seconds.
• Document patient visits using our intuitive Electronic Medical Record system.

Requirements

• Current Physical Therapist license issued by the state Board of Physical Therapy - The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Bilingual a plus!
