Patient Evaluation and Treatment

• Perform patient evaluations to assess physical condition and needs
• Create and implement individualized treatment plans
• Provide exercise instruction and pain management strategies
• Train patients and families on equipment usage
• Document patient progress accurately and timely

Team Collaboration

• Collaborate with nursing, therapy, and interdisciplinary care teams
• Support residents in regaining independence and quality of life

Requirements and Preferences

• Licensed in Hawaii or graduate of an approved Physical Therapy program
• Experience in post-acute care or similar healthcare environments preferred
