About the role

Chandler Creek Post Acute is a 133-bed skilled nursing facility in Greer, South Carolina. We are seeking a dedicated Physical Therapist (PT) to join our full-time rehab team.

Responsibilities

• Evaluate, develop, and implement individualized treatment plans to help residents improve mobility, reduce pain, and achieve independence.
• Provide therapeutic interventions to improve strength, mobility, and function.
• Maintain accurate and timely clinical documentation.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.

Qualifications

• Bachelor’s degree required; advanced degree preferred.
• Active, unencumbered Physical Therapist license in South Carolina.
• Experience in a long-term care or rehab setting preferred.
• Familiarity with PointClickCare and Casamba preferred.
• Strong communication and clinical decision-making skills.
• Passion for patient-centered care.
