About the Role

The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing environment, focusing on improving functional abilities and quality of life.

Responsibilities

• Develop and implement personalized treatment plans
• Assess patient progress and adjust care as needed
• Supervise therapy staff and students
• Document care accurately and maintain compliance with regulations
• Collaborate with interdisciplinary teams
• Ensure patient safety, professionalism, and confidentiality
• Participate in community health activities
• Maintain current licensure and CPR certification
• Follow treatment plans, set and adjust goals
• Oversee therapy assistants and students
• Chart appropriately and utilize therapy software
• Provide excellent patient service
• Demonstrate dependability, teamwork, and professionalism in a patient-centered setting

Requirements & Qualifications

• Graduate of an accredited physical therapy program
• Licensed in the state (new graduates welcomed)
• Proficient in Microsoft Office
• Excellent communication, body mechanics, and decision-making skills
• Knowledge of laws, regulations, and ethical standards
