Physical Therapist (PT)

About the role

The Physical Therapist provides direct rehabilitation care in physical therapy to patients in a skilled nursing facility setting, focusing on long-term care and short-term rehabilitation.

Responsibilities

• Develop, implement, and modify treatment plans based on physician referrals and patient needs.
• Assess patient progress, establish goals, and oversee therapy provided by PTAs, rehab aides, and students.
• Chart patient data accurately and utilize therapy software effectively.
• Collaborate with interdisciplinary teams, families, and support staff to ensure comprehensive patient care.
• Maintain patient safety, confidentiality, and adhere to all regulations and company policies.
• Participate in community health initiatives and uphold professional standards and conduct.

Qualifications

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
• Active licensure in the applicable state, maintained in good standing.
• Preferred experience in post-acute care or related settings.
• CPR certification (current or obtained during orientation).
• Proficiency in Microsoft Office and strong communication skills.
• Knowledge of laws, regulations, and best practices in post-acute care.

Essential Functions

• Follow treatment plans, evaluate patient progress, ensure proper documentation, and participate as a team member.
• Maintain professionalism, demonstrate good judgment, and provide excellent patient service.
• Physical stamina for patient mobility activities is required.
