The Physical Therapist (PT) provides direct rehabilitation care in physical therapy, assessing and implementing treatment plans for patients in a skilled nursing environment.

Responsibilities

• Establish and modify patient goals
• Oversee therapy staff and students
• Document treatment progress
• Utilize therapy software
• Ensure patient safety and satisfaction
• Collaborate with interdisciplinary teams
• Maintain confidentiality and adhere to legal and organizational standards
• Assist in emergency procedures
• Promote a positive, professional environment
• Follow treatment plans and evaluate care quality
• Chart accurately
• Participate in community health initiatives
• Maintain professional relationships
• Demonstrate integrity, professionalism, and dependable attendance in a patient-centered setting serving diverse populations with various post-acute needs

Requirements

• Degree from an accredited physical therapy program
• Active license in the state
• CPR certification
• Proficiency in Microsoft Office
• Strong communication skills
• Good body mechanics
• Ability to work independently
