Physical Therapist (PT)

About the Role

As a Physical Therapist, you will provide direct rehab care in physical therapy to patients upon physician referral, helping them achieve functional progress and improve quality of life within a supportive, patient-centered environment. You will work in a Skilled Nursing Facility (SNF) serving a diverse patient population with orthopedic, neurological, cardiopulmonary, and post-acute needs through long-term care and short-term sub-acute rehabilitation.

Responsibilities

• Follow physical therapy treatment plans for patients
• Establish, assess, and modify realistic, measurable, timely, and functional goals
• Oversee and evaluate care given by PTAs, Rehab Aides, and PT/PTA students
• Chart appropriately and timely
• Utilize therapy software accurately
• Exhibit excellent customer service and a positive attitude toward patients
• Assist in the evacuation of patients
• Participate in community health matters/projects as appropriate
• Liaison with patients, families, and support departments to plan for patient needs
• Implement and interpret department programs, goals, objectives, policies, and procedures
• Perform proficiently in competency areas including patient rights, safety, and sanitation
• Maintain professional working relationships with associates and vendors
• Maintain confidentiality of proprietary and confidential information
• Demonstrate integrity and professionalism by adhering to Life Care’s Code of Conduct

Requirements

• Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT; 2003 or later must have MSPT or DPT)
• Currently registered/licensed in applicable State; must maintain an active license in good standing
• One (1) year experience preferred; new grads welcome to apply
• CPR certification upon hire or obtain during orientation; must remain current
• Proficient in Microsoft Word, Excel, and email
• Good body mechanics and independent decision-making skills
• Knowledge of post-acute care facility laws, regulations, and guidelines
• Dependable, regular attendance
• Ability to concentrate, use reasoning skills, and good judgment
• Communicate and function productively on an interdisciplinary team
• Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
• Read, write, speak, and understand English

Pay

$45–$55/hour, depending on experience

Schedule

Full-time and PRN positions available. New graduates welcome.
