Physical Therapist (PT)

Evaluation

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.

Implementation

• Afford clinical support and instruct patients, families, and caregivers.

Documentation

• Measure & record patient’s motor function, strength, and muscle performance.
• Monitor patient responses to treatments and make modifications during treatment sessions as necessary.
• Complete documentation in accordance with company policy and state/federal regulations.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Licenses and Certifications

• Required licensure: Current License as a Physical Therapist in the State of PA. Must be in good standing.
