About the role

As a Home Health Physical Therapist (PT), you'll help patients regain their strength, manage their pain, and reach their goals in a home setting. Interim HealthCare, the nation's first home care company, understands the value of home-based care for individuals with strength and mobility challenges. You'll be part of a team that provides personalized therapy to fit patients' daily lives.

Responsibilities

• Complete initial and ongoing comprehensive assessments at appropriate time points.
• Absorb changes in the patient's status since the last visit/treatment day.
• Determine if the planned services need modification.
• Submit documentation of treatment and skilled intervention provided.
• Participate in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.

Requirements

• Current licensure or certification as a Physical Therapist active in Arizona.
• One (1) year Physical Therapy experience.
• Experience working in home care setting preferred.
• Valid Arizona Fingerprint Clearance card.
• CPR Certification (demonstration course required).
• No Covid Vaccination Required.
