Responsibilities

• Interprets referrals; examines, evaluates, develops, and modifies physical therapy plan of care, including anticipated goals and expected outcomes for patients.
• Performs comprehensive physical, psychosocial, and safety assessments of assigned patients as documented in clinical records.
• Coordinates patient care with other clinical team members to ensure optimal communication and care.
• Directs Physical Therapy Assistants in specific treatment components while remaining responsible for the patient’s episode of care.
• Rehab Only:
  • Provides and modifies skilled interventions using therapeutic procedures to produce changes consistent with diagnosis and prognosis.
  • Initiates corrective actions promptly when abnormal conditions or responses are noted.
  • Ensures written reports are concise, legible, accurate, and completed in a timely manner.
  • Assesses, interprets, and provides care based on the patient’s developmental life span and age-specific/cultural needs.
• Home Health Only:
  • Plans and provides skilled physical therapy treatment in the home, reassessing the patient’s status on each visit.
  • Maintains and submits all required patient documentation in accordance with home health policies.
• Reports to the assigned Manager or Director.
• May require additional physical demands or respiratory fit testing (consult Risk Management/Employee Health for details).
• Potential exposure to infections, communicable diseases, blood/body fluids, electrical equipment, and chemicals (standard precautions required).
• Aquatics Position: Frequent exposure to pool chemicals, heat, and humidity in the pool area.

Qualifications

• Graduate of an accredited school of Physical Therapy.
• Current Oklahoma Physical Therapy license OR eligibility for licensure in Oklahoma, with ability to practice under the Oklahoma Physical Therapy Practice Act. Must present proof of eligibility upon hire and obtain licensure within 6 months of initial temporary license issuance.
• Current BLS or CPR certification upon hire or within 30 days of employment.
• Ability to communicate effectively in English (verbal and written).
• Previous relevant work experience preferred.
• Department-Specific Preferences:
  • Aquatics: 2 years of experience and aquatic experience; water safety certification required within 6 months.
  • Hand Therapy: Certified Hand Therapist preferred.
  • Therapy Resource Pool: 2 years of experience preferred.
  • Community Re-Entry: 2 years of inpatient rehabilitation experience and ability to obtain a commercial driver’s license.
  • Jim Thorpe Therapy Resource Pool: Requires operation of an INTEGRIS-owned or personal vehicle; must have a valid Oklahoma driver’s license and an acceptable driving record per insurance carrier.
• Wound Care Departments: Color acuity required.
