Physical Therapist / PT

About the Role

Flagstar Nursing is seeking Physical Therapists (PT) for Skilled Nursing Facilities (SNFs) throughout NYC and Long Island. Work with a diverse, multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve patients' physicality and cognition, positively impacting their daily functioning. Opportunities are available for direct employment with our agency or as an independent contractor—we can assist with setting up a corporation free-of-charge.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver quality therapy services in an SNF setting for patients with conditions such as TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress using innovative EMR software.

Requirements

• Current NYS Physical Therapist License and Registration.
• CPR Certification.
• Competence with Electronic Medical Records (EMR).
• Meets all state and federal requirements for employment in a nursing facility.
• Malpractice/liability insurance (if applicable).

Pay

$46–$48 hourly (W-2) or $57–$60 hourly (1099).
