About the role

Flagstar Rehab is staffing Physical Therapists for the Rehab Department in state-of-the-art Skilled Nursing Facilities (SNFs) throughout NYC and surrounding areas. PTs work with a diverse, multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve patients' physicality and cognition, positively impacting their ability to function in daily life.

Flagstar Rehab offers the opportunity for direct employment with the agency or the option to be your own boss as an independent contractor. Assistance with setting up a corporation is available free-of-charge.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver quality therapy services to individuals in an SNF setting for conditions such as TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress using innovative EMR software.

Pay

• $46–$48 hourly (W-2 employee).
• $57–$60 hourly (1099 independent contractor).

Schedule

• Flexible options: full-time, part-time, per diem, weekends, coverage, days, and evenings.

Requirements

• Current NYS Physical Therapist license and registration.
• CPR certification.
• Competence with EMR software.
• Meets all state and federal requirements for employment in a nursing facility.
• Malpractice/liability insurance (if applicable).
