Physical Therapist (PT)

About the role

Full-time Physical Therapist providing direct rehabilitation care in a skilled nursing facility setting.

Responsibilities

• Develop and implement personalized treatment plans
• Monitor patient progress
• Supervise PTAs and students
• Document care accurately
• Collaborate with an interdisciplinary team to enhance patient outcomes
• Ensure safety
• Maintain patient confidentiality
• Deliver excellent customer service

Qualifications & Skills

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT)
• Current licensed/registered in applicable state and maintain licensure in good standing
• Preferably one year of post-acute care experience
• CPR certification (must be current)
• Proficient in Microsoft Office and communication skills
• Knowledge of laws, regulations, and standards related to post-acute care
• Ability to make independent decisions and demonstrate professionalism and integrity

• Follow treatment plans, establish and modify goals
• Evaluate care provided
• Chart promptly
• Utilize therapy software
• Participate in community health projects
• Maintain excellent teamwork, uphold safety and confidentiality standards
• Demonstrate dependable attendance
