About the role

Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

Responsibilities

  • Complete comprehensive in-home assessments and deliver skilled PT visits.
  • Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
  • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
  • Manage treatment and outcomes directly, including PTA supervision where appropriate.
  • Coach patients and caregivers with practical home programs that actually get done.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

  • Active Texas PT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver’s license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
  • Strengthening, functional mobility, and home safety assessment/modification education
  • Patient/caregiver coaching with practical home programs

Qualifications

Must-haves:

  • Active Texas PT license (in good standing)

  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper clinical experience in high-signal PT populations
  • Experience supervising/mentoring PTAs and collaborating on care plans
  • Strong organization and time management
