About the role
Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
Complete comprehensive in-home assessments and deliver skilled PT visits.
Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
Manage treatment and outcomes directly, including PTA supervision where appropriate.
Coach patients and caregivers with practical home programs that actually get done.
Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
Responsibilities

Requirements
- Active Texas PT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PT scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
Qualifications
- Nice-to-have: Prior home health experience
- Nice-to-have: Homecare Homebase (HCHB) experience (or similar home health EMR)
- Nice-to-have: Deeper clinical experience in high-signal PT populations
- Nice-to-have: Experience supervising/mentoring PTAs and collaborating on care plans
- Nice-to-have: Strong organization and time management

Benefits
- Flexible schedule: Full-time Monday–Friday with occasional weekends (as needed)
- Competitive compensation: $75/point - 130k/year
