What You Will Do
- Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
- Complete comprehensive in-home assessments and deliver skilled PT visits.
- Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
- Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
- Manage treatment and outcomes directly, including PTA supervision where appropriate.
- Coach patients and caregivers with practical home programs that actually get done.
- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
Requirements
- Must-haves:
- Active Texas PT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Credential in core home health PT scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
- Nice-to-haves:
- Prior home health experience
- Deeper clinical experience in high-signal PT populations
- Experience supervising/mentoring PTAs and collaborating on care plans
- Strong organization and time management
