Key Responsibilities
- Evaluate individuals across all populations and develop individualized physical therapy treatment plans
- Implement therapy programs to improve function, alleviate pain, and prevent dysfunction
- Maintain thorough and accurate documentation, including evaluations, daily notes, progress reports, and discharge summaries
- Educate patients, families, and caregivers on care strategies and home exercise programs
- Assess treatment effectiveness and adjust interventions to achieve optimal outcomes
- Evaluate patient environments (home, daycare, community) and recommend modifications to support functional independence
- Collaborate with interdisciplinary team members to address patient needs
- Manage caseloads, coordinate scheduling, and ensure accurate billing practices
Qualifications
- Doctorate Degree in Physical Therapy from an accredited program
- Current Arizona Physical Therapy license required
- Experience in pediatrics preferred, but not required; new graduates are encouraged to apply
- Ability to work with both pediatric and adult populations
- Strong communication and interpersonal skills
- Compassionate, reliable, and adaptable approach
Specialty & Program Areas
- Assist with Custom Wheelchairs (PT/OT collaboration)
- Assist with Custom Durable Medical Equipment (DME) (OT/PT collaboration)
- Support Feeding Services (High-Need Area) in collaboration with OT/SLP
- Deliver Community-Based Services (marketed as community services, not home health) across all disciplines
- Deliver Telehealth Services across all disciplines
Pay
PRN Pay: up to $65 per hour, based on experience
