Physical Therapist - PRN

About the Role

Provide in-home physical therapy services by assessing patient needs, developing treatment plans, and implementing interventions to improve mobility, reduce pain, and restore or maintain functional abilities.

Responsibilities

• Evaluate patients' functional status and assist in developing therapy plans of care.
• Deliver services within the scope of practice, collaborating with the healthcare team.
• Observe and report patient responses and condition changes.
• Treat patients to improve or maintain function using established standards.
• Educate patients, families, and team members about care plans, limitations, and progress.
• Complete patient evaluations, progress notes, and reports on outcomes and care frequency.
• Function effectively in diverse home environments, respecting all patients regardless of background.

Requirements

• Current physical therapy licensure in the practicing state.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.

Preferred Qualifications

• CPR certification or ability to obtain within 90 days.
