Physical Therapist - PRN

About the role

A PRN Physical Therapist (PT) is needed to evaluate and treat residents to enhance their mobility, strength, and independence.

Responsibilities

• Perform comprehensive assessments of residents' physical abilities and needs.
• Develop and implement personalized treatment plans focused on improving function, balance, and mobility.
• Deliver therapeutic interventions such as exercises, manual therapy, and assistive device training.
• Monitor progress and adjust treatment plans accordingly.
• Collaborate with an interdisciplinary team to ensure holistic resident care.
• Educate residents and families on therapy goals and exercises for continued improvement.
• Accurately document evaluations, sessions, and progress notes per standards.
• Participate in quality improvement initiatives within the rehabilitation department.

Qualifications

• Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program.
• Current and valid PT license in the state.
• Experience in skilled nursing or rehab settings preferred.
• Strong communication, teamwork, and resident-centered approach.
• Ability to work independently and adapt to a flexible schedule.
