About the role
A PRN Physical Therapist (PT) is needed to evaluate and treat residents to enhance their mobility, strength, and independence.
Responsibilities
- Perform comprehensive assessments of residents' physical abilities and needs.
- Develop and implement personalized treatment plans focused on improving function, balance, and mobility.
- Deliver therapeutic interventions such as exercises, manual therapy, and assistive device training.
- Monitor progress and adjust treatment plans accordingly.
- Collaborate with an interdisciplinary team to ensure holistic resident care.
- Educate residents and families on therapy goals and exercises for continued improvement.
- Accurately document evaluations, sessions, and progress notes per standards.
- Participate in quality improvement initiatives within the rehabilitation department.
Qualifications
- Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program.
- Current and valid PT license in the state.
- Experience in skilled nursing or rehab settings preferred.
- Strong communication, teamwork, and resident-centered approach.
- Ability to work independently and adapt to a flexible schedule.
