Physical Therapist - PRN
Overview
If you’re a Physical Therapist (PT) who wants to empower patients to achieve meaningful outcomes through compassionate and patient-centered therapy—this is it. In this role, you’ll provide physical therapy services to the adult/geriatric population, working closely with patients to improve independence, mobility, safety, and overall quality of life. You’ll have the opportunity to follow patient progress over time and deliver truly patient-centered care in a collaborative clinical environment.
Responsibilities
- Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence.
- Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
- Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults.
- Help patients restore functional mobility and safely perform daily activities with greater confidence and independence.
- Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care.
- Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals.
Qualifications
- Licensed Physical Therapist (PT) or license eligible in the state of practice.
- Graduate of an accredited Physical Therapist Program.
- Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
- Passion for patient-centered care and improving quality of life for older adults.
