About the role
We are looking for a Physical Therapist to join our team on a PRN basis - ideally 1 to 2 weekdays a week in a skilled nursing setting.
Responsibilities
- Implement residents' personal care plans, focusing on residents' strengths, preferences, and preferred routines
- Support residents' physical, spiritual, emotional, and clinical needs
- Work with the interdisciplinary clinical team to address the needs of our residents
- Provide a holistic approach to care
Requirements
- Must possess a valid and current state Physical Therapy license
- Experience in older adults across multiple diagnostic groups is preferred
Qualifications
- Must meet current Medicare and state requirements
Skills
- Therapeutic experience in older adults
