Physical Therapist, PRN

About the role

Provide patient-centered physical therapy services in a home-health setting based in Reading, PA. Assess and develop treatment plans, deliver care within the community, and collaborate with patients and team members to promote optimal health outcomes.

Responsibilities

• Conduct patient assessments
• Implement individualized treatment plans
• Document progress and ensure quality care delivery

Requirements

• Minimum nine months of physical therapy experience; community or home-health experience preferred
• Master’s degree in physical therapy from a CAPTE-accredited program
• Current state license to practice physical therapy
• Valid CPR certification
• Valid driver’s license for travel
