Physical Therapist (PRN) - Ahwatukee Post Acute

About The Opportunity

Ahwatuke Post Acute is growing — and we are seeking a dedicated Physical Therapist to join our in-house therapy team! Our 192 bed facility is currently undergoing a major renovation, including construction of a state-of-the-art 44 bed behavioral and dementia care unit, enhancing our ability to provide specialized services to the Phoenix and Ahwatukee communities. We proudly maintain a CMS 5-Star Quality Measure rating and foster a supportive, collaborative clinical environment focused on patient-centered success.

What You’ll Do

• Perform skilled PT evaluations and provide evidence-based treatment
• Develop individualized care plans to increase function, safety, and mobility
• Document patient progress using Net Health
• Collaborate closely with nursing and IDT using PointClickCare
• Educate patients, families, and caregivers on therapy strategies

Qualifications

• Az PT license (or eligible prior to start date)
• Strong communication and teamwork skills
• A passion for working with the geriatric and memory care population
