About the role
The Physical Therapist provides quality care for patients receiving Physical Therapy services, including examination, evaluation, Physical Therapy diagnosis, prognosis, intervention, outcomes, and discharge planning. Additional responsibilities include consultation, education, critical inquiry, administration, and direction and supervision of personnel.
Responsibilities
- Provides physical therapy services to patients with movement disorders.
- Consults with patients, their primary care providers, and specialists to ensure high-level care.
- Supports program goals, Medical Center goals and objectives, functions, and philosophy through an individualized performance development plan.
- Works closely with Primary Care Providers and specialists to facilitate a smooth transition to the care needed.
Requirements
- Bachelor's Degree in Physical Therapy.
- Graduate of an accredited Physical Therapy professional program.
- Certified California Physical Therapist License.
- Within 30 days of hire: National Provider BLS - American Heart Association.
- New Grad or entry level experience in outpatient Physical Therapy setting.
- Experience with program planning, implementation, evaluation, and education training.
Preferred Qualifications
- Master's Degree or Ph.D.
- One year of experience in outpatient Physical Therapy setting.
- Experience with CARF and Title XXII standards.
- EMR experience.
