Physical Therapist - Per Diem

We are seeking a licensed Physical Therapist (PT) to provide in-home therapy services within a community-based healthcare setting. Key Responsibilities Include Conducting patient evaluations to assess needs and develop individualized treatment plans.Providing therapy interventions using appropriate protocols and techniques to improve patients' physical, social, and mental functions.Establishing and communicating care and discharge plans to relevant parties.Completing accurate clinical documentation within designated timeframes.Supervising assigned therapy assistants and performing additional related duties as needed. Qualifications And Skills Include Doctor of Physical Therapy (DPT) degree or foreign equivalent; licensure in applicable states.Valid Physical Therapy licensure, CPR/BLS certification, and a valid driver’s license.Strong clinical evaluation and treatment skills, excellent communication, and organizational abilities. Additional Requirements Commitment to safety protocols and vaccination policies.Ability to work independently within an interdisciplinary team environment.
