Physical Therapist - Part Time

About the role

We are seeking a licensed Physical Therapist to provide home health care services.

Responsibilities

• Assess patients' functional status, mobility, pain, and medical conditions.
• Develop and implement individualized treatment plans in collaboration with healthcare teams.
• Provide interventions to improve or maintain patient well-being and progress.
• Monitor and report patient responses and condition changes.
• Educate patients, families, and caregivers about treatment plans and progress.
• Complete patient evaluations, progress notes, and reports as per policy and state guidelines.
• Adapt to diverse home environments and patient needs.

Requirements

• Current state licensure in Physical Therapy.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Preferred CPR certification or ability to obtain within 90 days.
• Strong clinical assessment and communication skills.
• Ability to work independently and adapt to various home settings.
