Physical Therapist - Part Time

• Assessing participants’ needs through in-person evaluations and ongoing monitoring.
• Developing and implementing personalized treatment plans in accordance with recognized standards and policies.
• Administering therapy treatments, measuring effectiveness, and adjusting plans as necessary.
• Educating participants and their families on procedures, exercises, and safety measures.
• Recommending adaptive equipment, home modifications, and assessing safety in participants’ environments.
• Training staff in proper body mechanics, equipment use, transfers, and gait patterns to maximize safety and independence.
• Collaborating with the interdisciplinary team to develop comprehensive care plans and participating in team meetings.
• Documenting evaluations, treatments, and participant progress accurately and timely.
• Participating in quality improvement initiatives and organizational meetings.
• Ensuring compliance with safety policies, confidentiality, and OSHA guidelines.

Qualifications and Skills

• Advanced degree in Physical Therapy from an accredited institution.
• Current state license to practice Physical Therapy.
• Valid driver’s license and auto insurance proof.
• Preferably, two years of experience in interdisciplinary team settings and one year working with elderly or frail populations.
• Strong communication, documentation, and collaborative skills.
• Commitment to ongoing professional development and adherence to safety standards.
