Required qualifications

• Current Doctor of Physical Therapy degree from an accredited program
• Active Colorado PT license (Compact accepted)
• BLS certification (copy needed)
• SSN required
• DOB required
• 1 reference in entire work history

Preferred qualifications

• One year of recent outpatient experience recommended
• License and OP experience are required
• Travel experience is preferred
