Responsibilities
- Develop and implement individualized treatment plans
- Work closely with patients and families
- Coordinate with other healthcare professionals to ensure quality care
- Assess patient needs and conduct therapy sessions
- Document progress and adjust treatment plans as needed to help patients achieve their rehabilitation goals
Requirements
- Physical Therapist certification
- BLS certification (copy needed)
- SSN and date of birth required
- One reference from entire work history
- State license and outpatient experience required
