Role Overview

Responsible for coordinating and delivering physical therapy services, developing individualized treatment plans, and implementing strategies to promote patient health and recovery.

Key Responsibilities

• Evaluate patients through various methods and develop tailored treatment programs.
• Use therapeutic activities to achieve treatment goals and adjust plans based on ongoing assessments.
• Provide direct therapy services in different settings, coordinating with medical teams and families.
• Document evaluations, treatment plans, progress notes, and communicate effectively with patients and team members.
• Build rapport and collaborate with interdisciplinary teams to support patient care.
• Participate in ongoing education to enhance professional growth.

Qualifications & Skills

• Active license as a Physical Therapist in the practice state.
• Preferred 1-3 years of experience as a licensed physical therapist.
• Effective communication, visual acuity, and ability to work in a team environment.
• Commitment to confidentiality, safety standards, and organizational values.
