Physical Therapist/ Occupational Therapist lymphedema specialist

About the role

As a Physical Therapist in our Skilled Nursing setting, you will evaluate and treat patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. You will develop individualized treatment plans, collaborate with the interdisciplinary care team, and play a direct role in helping patients return to their highest level of independence.

Responsibilities

• Evaluate and treat patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning.
• Develop individualized treatment plans.
• Collaborate with the interdisciplinary care team.
• Play a direct role in helping patients return to their highest level of independence.
• Conduct comprehensive occupational therapy evaluations.
• Develop individualized treatment plans focused on functional independence.
• Collaborate with nursing, PT, SLP, and social work to ensure coordinated patient care.
• Address self-care skills, cognitive and perceptual deficits, home safety, adaptive equipment needs, and caregiver training.

Requirements

• Doctor of Physical Therapy (DPT) or Master's degree from an accredited PT program.
• Master's or Doctoral degree in Occupational Therapy from an accredited program.
• Current or eligible for New York State PT/OT license.
