About the Role

We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities, utilizing a Geriatric House Calls™ model. The role involves performing evaluations, developing personalized treatment plans, delivering evidence-based therapy, and promoting patient independence and mobility. Therapists may incorporate telehealth and virtual care to enhance flexibility and patient access. Collaboration with an interdisciplinary team and patient education are key responsibilities.

Responsibilities

• Deliver individualized physical therapy sessions in various settings
• Document care effectively and adhere to regulatory standards
• Educate patients, caregivers, and staff on safety and aging in place
• Utilize telehealth/virtual care when appropriate
• Support patient wellness, mobility, and functional independence

Requirements

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Proficiency with EMR systems
• Current CPR certification
• Ability to meet physical demands such as lifting, assisting with transfers, and driving
• Basic computer skills
