Physical Therapist, Meade Street Skilled Nursing Center (Call-In)

Brief Description

The Physical Therapist reports to the Rehab Director and provides physical therapy programs within the SNC department. Responsibilities include:

• Providing care appropriate to adult through geriatric patients
• Demonstrating knowledge and skills necessary for age-specific patient care
• Absessing data reflective of patient status and interpreting necessary information for patient care
• Identifying patient requirements based on age-specific needs
• Implementing care according to Physical Therapy Department policies and procedures

Education

• Bachelor’s degree in Physical Therapy from an APTA-approved school required
• Master’s degree in Physical Therapy from an APTA-approved school preferred
