About the role

As a Physical Therapist at Family Life Care, you will play a vital role in managing the care of our patients by providing compassionate and professional care in the comfort of their own home. FLC serves the entire community, from children to senior citizens, offering opportunities to choose from various assignments including patient case management, long-term care, and assisting with recovery from injuries or illnesses.

Responsibilities

• Patient assessment, admission, and coordination of care with other disciplines, physicians, and care managers
• Provide Physical Therapy and treatment of home care patients in accordance with the plan of treatment and with company policies and procedures
• Provide Physical Therapy in a pleasant, professional manner to maximize positive outcomes and patient satisfaction
• Ensure that all clinical assessments, care plans, starting and resumption of care are completed with agency guidelines
• Train, coach, and direct clinical staff in the performance of their clinical duties
• Responsible for compliance with all state and federal laws, regulations, policies, and processes relevant to home health

Requirements

• Current Florida Physical Therapy license
• CPR Certification
• Driver’s License and Vehicle Insurance
• Current TB test with negative results
• Ability to pass a Level II background check
