Physical Therapist I - AVN, Per Diem, Sussex County

About the role

Atlantic Health is seeking a seasoned Per Diem Physical Therapist for the dynamic Atlantic Visiting Nurse Dept to work 8am-4pm in beautiful Sussex County.

Responsibilities

• Conduct comprehensive evaluations and develop patient-centered treatment plans in the home of each patient
• Collaborate with physicians, case managers, and fellow rehab team members
• Communicate with patients and families to ensure understanding and continuity of care
• Support various appointment types and adapt care plans as needed

Requirements

• Minimum Education: High School Diploma or GED, required Bachelor's in Physical Therapy, required Master or Doctorate in Physical Therapy preferred
• Minimum Experience: 1 year of experience in homecare, strongly preferred
• Licenses/Certifications: NJ License in Physical Therapy, required BLS, preferred Valid and unrestricted Driver License with a clean driving record, required

• Essential Functions/Responsibilities: Evaluate and treat patients in their homes
