Physical Therapist - Home Health Therapy

We’re looking for an experienced Physical Therapist to join our Home Health team in a full-time capacity. This opportunity is ideal for a PT who values autonomy, relationship-building, and delivering meaningful care where patients are most comfortable — at home. Responsibilities

• Provide skilled physical therapy services to home health patients across the Omaha metro area
• Develop individualized treatment plans focused on mobility, strength, safety, and independence
• Partner closely with nurses, providers, caregivers, and interdisciplinary teams to support patient goals
• Complete timely documentation and maintain compliance with all regulatory requirements
• Educate patients and families on exercises, safety, and continued care plans
• Build trusted relationships with patients while delivering compassionate, high-quality care

Requirements

• Licensed Physical Therapist in the state of Nebraska (or eligible for licensure)
• Previous home health experience strongly preferred
• Minimum of 1+ years of PT experience required
• Strong clinical judgment, communication, and organizational skills
• Ability to work independently while staying connected with the care team
• Reliable transportation and valid driver’s license
