KeystoneCare is seeking a dedicated Physical Therapist to join our Home Health team in the Philadelphia Area. This position offers an exciting opportunity to provide personalized rehabilitation services to patients in the comfort of their homes, ensuring they receive the best therapeutic care tailored to their individual needs.

Responsibilities

• Assess, plan, and execute treatment programs focusing on improving mobility, strength, and overall functional abilities in patients of all ages.
• Collaborate closely with a multi-disciplinary team to enhance the quality of life for our patients, guiding them through their recovery journey with compassion and professionalism.
• Play a key role in restoring patients' independence and well-being, making a meaningful impact on their lives.

Requirements

• Graduate of an accredited Physical Therapy program
• Current Pennsylvania Physical Therapy License
• Minimum two years of experience in Home Health or similar settings
• Valid Driver's License and reliable vehicle
• Proof of Auto Insurance
• Strong communication skills for effective collaboration with patients, families, and the care team
• Excellent organizational abilities and capacity to manage a diverse caseload

License/Certification

• PA Physical Therapy License (Required)
