Actify Home Care has active PT home health cases ready for assignment across our expanding territory in North Palm Beach County (Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach, and surrounding areas). We can fast-track your credentialing and get you working quickly. Actify Home Care is clinician-owned and operated by a Physical Therapist with 20 years of experience, offering a clinical team that understands the physical and clinical demands of field‑based PT therapy.

Responsibilities

• Provide high‑quality, one‑on‑one physical therapy as a field PT to geriatric patients in their homes.
• Implement effective plans of care to improve patient mobility and safety.
• Complete Evaluations, Re‑Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with support from the clinical team.
• Collaborate with clinical leadership, office staff, and fellow therapy assistants to deliver outstanding patient care.

Requirements

• Active Florida Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.
• Kinnser/WellSky experience is a major plus.

Pay

• Start of Care (SOC): $110.00 per visit (1099)
• Evaluation / Re‑Admission: $80.00 per visit (1099)
• Routine Visit: $70.00 per visit (1099)
