About the Role

Provide in-home physical therapy services by evaluating patients' functional status, mobility, endurance, and pain. Develop and implement personalized treatment plans to improve or maintain patient well-being, and monitor progress.

Responsibilities

• Conduct patient assessments
• Assist in developing therapy plans
• Deliver treatment within legal scope
• Observe and report patient responses
• Educate patients, families, and team members
• Document visits and progress
• Adapt care to diverse home environments

Qualifications & Skills

• Current state licensure
• Valid driver’s license
• Vehicle insurance and access to transportation
• CPR certification preferred
• Ability to work independently and collaboratively
• Strong communication and clinical skills

Pay

Compensation includes per visit and hourly pay.
