Physical Therapist, Home Health
About the role
As a full-time therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
Responsibilities
- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Requirements
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
Preferred Qualifications
- One year of experience as a Physical Therapist
Pay
The pay range is $59.00 - $83.00 per visit/unit, with a base pay of $93,000 - $128,000 per year.
Schedule
This is a full-time position with 40 scheduled weekly hours.
